For your records, our Tax ID # is 95-4787248.<\/p>\n<div>\n<p><strong>About Foothill Unity Center<\/strong><br \/>\n<em>Now in its 34th year of serving people in crisis with love and dignity, Foothill Unity Center is the main provider of food, case management, crisis help and access to health care resources across eleven San Gabriel Valley cities. 77% of its clients are at or below the National Poverty Level. Last year, the center distributed over 4.23 million pounds of food to over 4,600 non-duplicated local families (over 1,200 individuals). The number of people the Center serves has more than tripled over the last five years. We are now preparing to provide holiday meals, toys and more for over 1,800 families and 3,000 children.<\/em><\/p>\n<p><strong>Foothill Unity Center: Overview of 2013 Services<\/strong><\/p>\n<p>Last year, Foothill Unity Center served<strong> 4,220 <\/strong>unduplicated local, low-income families, (<strong>10,472<\/strong> persons) from Monrovia, Arcadia, Duarte, Sierra Madre, Bradbury, Pasadena, Altadena, South Pasadena, Irwindale, Baldwin Park and Azusa at no cost to the family. 79% of our clients were at or below the National Poverty Level.\u00a0The rest were less than 150% above that level.<\/p>\n<p><strong>Basic Needs<\/strong><\/p>\n<ul>\n<li>Provided <strong>61,806<\/strong> client services including <strong>35,190 <\/strong>food services<\/li>\n<li>Delivered monthly food to <strong>72 \u00a0homebound<\/strong>\/disabled families\/seniors<\/li>\n<li>Registered <strong>1,452<\/strong> children to be <strong>Back-to-School <\/strong>ready with school supplies, clothing, haircuts and more<\/li>\n<li>Prepared\u00a0 bountiful <strong>Thanksgiving<\/strong> Dinner food boxes for <strong>1,116 <\/strong>families<\/li>\n<li>Prepared <strong>1,307<\/strong> <strong>Holiday <\/strong>family food boxes and <strong>new toys, clothing, and much more<\/strong> for <strong>1,717<\/strong> children<\/li>\n<li>Gave <strong>4,749<\/strong> bag lunches\/meals to <strong>homeless<\/strong> men, women and children<\/li>\n<li>Issued <strong>5,813 <\/strong>Bus Tokens\/Taxi Vouchers\/Foothill Transits to remedy transportation barriers<\/li>\n<li>Provided <strong>100<\/strong> emergency shelter <strong>motel vouchers<\/strong> for families in-crisis<\/li>\n<li>Provided <strong>2,873<\/strong> individuals with clothing and\/or household items, registered <strong>66<\/strong> families for CalFresh\/food stamps<\/li>\n<li>Connected <strong>764<\/strong> people to additional services in their areas through Information and Referral calls<\/li>\n<li>Provided transitional and supportive services for victims of domestic violence and family support programs<\/li>\n<li>Helped families secure permanent housing through move-in and eviction prevention assistance<\/li>\n<\/ul>\n<p><strong>Health Services<\/strong><\/p>\n<ul>\n<li>Gave<strong> 203 Dental<\/strong> <strong>Screenings <\/strong>and <strong>212<\/strong> <strong>Vision Screenings <\/strong>to children<strong> <\/strong>at our Back-to-School event<\/li>\n<li><strong>Mobile Dental Clinic<\/strong> with comprehensive dental treatment for <strong>96<\/strong> children; <strong>2,294<\/strong> procedures<\/li>\n<li><strong>Children\u2019s Health Connections (CHC<\/strong>) program staffed by<strong> 27<\/strong> professional healthcare volunteers provided free health care to <strong>150<\/strong> uninsured and underinsured children<\/li>\n<li><strong>Eye exams <\/strong>and <strong>glasses<\/strong> provided for <strong>29 <\/strong>adults and <strong>55<\/strong> children<\/li>\n<li>Gave<strong> 281 medical exams<\/strong> to homeless and uninsured clients via our <strong>Medical Outreach Program<\/strong><\/li>\n<li><strong>Enrolled 18 <\/strong>children<strong>\/12 <\/strong>families for Healthy Families \/Medi-Cal Coverage<\/li>\n<li><strong>Gave 179 flu vaccines <\/strong>and <strong>73 Diabetes <\/strong>and<strong> Blood Pressure Screenings<\/strong><\/li>\n<li>Referred <strong>142<\/strong> client families<strong> <\/strong>to<strong> Smoking Cessation <\/strong>programs<\/li>\n<\/ul>\n<p><strong>Staffing<\/strong><\/p>\n<p>Our staff consists of only <strong>25<\/strong> full and part-time employees and <strong>2 <\/strong>AmeriCorps\u2019 Duarte\u2019s Promise staff. Dedicated volunteers, trainees and interns perform much of the work required to run the Center\u2019s services. <strong>3,066<\/strong> volunteers ranging in age from 3 to 96 from local and neighboring schools, faith-based organizations, businesses, service and government groups contributed <strong>45,407<\/strong> hours of their time, energy, respect, and love in 2013.
